TeamHealth offers a full benefits package to full-time employees. Generally, benefits are effective on the first of the month following 30 days of employment. This website is designed to provide an overview of benefits available.
TeamHealth offers an award-winning, comprehensive wellness program, LiveWell, to employees and family members to help participants be health and stay healthy. The 2014 Health Risk Premium begins October 1, 2014 and applies to employees and covered spouses on the UnitedHealthcare medical plan. The $800 Health Risk Premium will be assessed to employees and covered spouses who do not meet all five wellness goals and submit an exemption form by August 31, 2014. The Benefits Year is October 1 through September 30. Open Enrollment occurs in August, and any changes to benefits outside of this enrollment period require a qualifying event. If you have a qualifying event and you would like to request a change to your benefits, complete the Qualifying Event Notification Form.
