Short-Term and Long-Term Disability
TeamHealth offers disability insurance to continue income if an associate cannot work due to sickness or injury. Short-term disability insurance is an employee-paid, voluntary benefit. The benefit pays 60 percent of weekly benefit salary for up to 180 days (the elimination period for long-term disability)
Disability and FMLA requests are administered through Unum. Contact Unum directly at 866-779-1054 with any questions. Spectrum Healthcare Resources, Mid-Ohio Emergency Services and IPC associates should call their Human Resources Representative for FMLA and will call Unum for short-term disability.
Life and Accident Insurance
TeamHealth provides term life and accidental death and dismemberment insurance through Unum. Supplemental life and accidental death and dismemberment insurance is also available for associates, their spouses and children.
Group Term Life
Group term life and accidental death and dismemberment insurance is provided to full-time associates at one times benefit salary.
